In order to join us, you must submit both Fountainhead and State of California required forms and materials. Please see the checklist below for required forms and materials, or print a hard copy of our enrollment checklist.
Required:
- Booklet of forms
- Physician's report
- Copy of immunization record
- Financial contract, which will be mailed to each family upon enrollment
- Emergency and earthquake card
- A small color picture (5x7 or smaller), two for Preschool through Kindergarten classes, one for Toddler and Prep classes
- Change of clothes, labeled
Optional
- Diapers, if your child is not yet toilet trained
- Nap gear, including a crib-sized fitted sheet, blanket, and pillow if your child will be napping
- Electronic Payment Authorization, required for biannual contract families
- A schedule indicating your childcare needs, if you plan on using childcare on a regular basis
- Our passive fund-raising packet, if you'd like to help support Fountainhead
- A medication release, for any prescriptions to be administered on-site
